Welcome to the workshop – Implement and Monitor Care for a Person with Chronic Health Problems
This workshop describes the skills and knowledge required to implement and monitor care for a person with chronic health problems, including supporting ongoing care needs, monitoring changes in health status, and responding appropriately in line with individual care plans and workplace procedures.
